Village Primary Care
in Hoosick Falls, participates in the following insurances in addition to many of the same ones that we participate in:

- New York Medicaid

Even though our office does not directly participate with these insurances, if you are seen by us on an evening or weekend when the Village office is closed, your insurance should cover the costs of the visit to our office as covering physicians for the Village office.

Referrals to Specialists
If you need to see a specialist, and your insurance requires a referral from your primary care provider, please proceed as follows:
RENEWAL: For a return visit to a specialist whom you have seen before for a health problem for which that specialist has already been seeing you, you will soon be able to send us electronically our
a request form.
We will post an announcement on our
announcements
page when the e-form becomes available.
NEW: For a first time visit to a new specialist, or to see a specialist whom you have seen before, but for a new health problem, you must call, and/or make an appointment to be seen by, your primary care provider.
